How many times have you heard, “If it wasn’t charted, it wasn’t done?” We all know this isn’t actually the case, but giving good care isn’t always enough when you’re involved in an investigation or lawsuit. An entire case can be decided by the way your staff has described the care it has given. The quality of your charting (or the lack thereof) will either be your best weapon or your worst enemy.
In this fun and informative presentation, trial attorney Alana Bassin from Bowman and Brooke will discuss common pitfalls in charting and documentation, and, more importantly, explain how poorly chosen words on a chart can undo years of quality care. Ms. Bassin will provide numerous examples of how language, lack of language, inconsistency in charting and other documentation errors can create obstacles to building a strong defense, as well as provide tips on how to chart effectively to support the fact that a resident received excellent care. This presentation will be of special importance to Administrators, DONs, and Nurses.
